Job Summary

We are seeking a skilled General Practitioner to join our Community Hospital Rehabilitation team at Oxford Health NHS Trust. As a GP for Community Hospital, you will play a vital role in providing medical care to patients in our community hospitals across Oxfordshire.

Main Duties

The successful candidate will be responsible for:

  • Proactive assessment of new admissions to the ward, including medical clerking, medication review, and prescribing.
  • Performing Comprehensive Geriatric Assessments alongside the multidisciplinary team to optimize patient health and create advanced care plans.
  • Responding to medical concerns, including history taking, examination, investigation, diagnosis, treatment, and onward referral when necessary.
  • Participating in weekly multidisciplinary team meetings and ward rounds.
