We are seeking a highly skilled and experienced Discharge Liaison Practitioner to join our multidisciplinary team at Southern Health NHS Foundation Trust.
Key Responsibilities- Support the safe discharge of individuals from acute mental health units.
- Provide a 3-month period of monitoring in the community with a clear care plan.
- Review mental health needs and provide focused interventions to support recovery.
- Enable a safe transfer of care to primary care as appropriate.
- Refer for CMHT Care Coordinator allocation if longer-term mental health needs are identified.
- Make onward referrals and/or signposting to appropriate agencies.
- Registered Nurse, Social Worker, Mental Health Practitioner or equivalent qualification.
- Significant post-qualifying experience in mental health.
- Computer skills and proficiency in relevant software.
- Mentorship course and clinical supervision training.
