Job Title: Senior Community Nurse Specialist

We are seeking an experienced and skilled Senior Community Nurse Specialist to join our Out of Hours Nursing Team within Community Health and Social Care. As a key member of our team, you will be responsible for leading the team for the shift, triaging referrals for unplanned community nursing needs, and supporting and clinically advising team members in managing patients' care at home out of hours.

Main Duties:
  • Lead the Out of Hours Nursing Team for the shift, ensuring effective and efficient delivery of care to patients.
  • Triage referrals for unplanned community nursing needs, prioritizing and allocating tasks to team members as necessary.
  • Support and clinically advise team members in managing patients' care at home out of hours, providing guidance and expertise as required.
  • Contribute to the development and implementation of care plans, ensuring that patients receive high-quality, person-centered care.
  • Collaborate with other healthcare professionals and agencies to ensure seamless care coordination and delivery.
Requirements:
  • Registered Nurse with a valid NMC registration.
  • Experience in community nursing, with a strong understanding of community health and social care principles.
  • Excellent leadership and communication skills, with the ability to motivate and support team members.
  • Strong clinical skills, with the ability to assess and manage patients' needs effectively.
  • Ability to work flexibly, including evenings, nights, and weekends, as required.
