SEND Transition Coordinator

We are seeking a highly skilled and experienced SEND Transition Coordinator to join our Learning Support team at Uxbridge College Careers. As a key member of our team, you will play a vital role in supporting young people with Special Educational Needs and Disabilities (SEND) to transition successfully to our college.
Key Responsibilities- Develop and maintain effective relationships with young people, parents, and carers to ensure a smooth transition process.
- Collaborate with schools, specialist providers, and local authorities to arrange interviews, visits, and taster sessions.
- Provide guidance and advice to learners and parents on the college's curriculum offer, travel arrangements, and individual needs.
- Administer the formal review process for learners with Education, Health and Care Plans.
- Contribute to the development and implementation of strategies to support learners with SEND.
- Proven experience of working with children or young people with SEND in an educational environment.
- Excellent communication and interpersonal skills, with the ability to build effective relationships with diverse stakeholders.
- Good IT, organisational, and administrative skills, with the ability to prioritise a busy workload.
