This is an exciting opportunity to join our Community Hepatology Nursing Team as a Hepatology Community Liaison Nurse. You will work closely with our Nurse Manager and wider team to deliver high-quality care to patients with Hepatitis C across the Thames Valley Operational Delivery Network (ODN).
Main Duties- Develop and implement clinical pathways for Hepatitis C management in various settings, including testing campaigns, outreach clinics, GP surgeries, and prisons.
- Work closely with the community team to provide comprehensive care to patients with Hepatitis C in the Milton Keynes area.
- Collaborate with the multidisciplinary team to develop and deliver training and development programs for healthcare professionals.
- Contribute to the development of new initiatives and ideas to improve Hepatitis C care and services.
