About the Role

The Bury Council is seeking a highly skilled and dedicated Adult Social Worker to join the Hospital Team. As a key member of the Integrated Discharge Team, you will play a vital role in facilitating safe and timely discharges of patients from the hospital.

Key Responsibilities
  • Undertake statutory Social Work functions in line with the Care Act and the Mental Capacity Act.
  • Support patients and their families to promote independence, choice, and control.
  • Work collaboratively with multi-disciplinary teams to achieve positive outcomes.
  • Develop and implement personalized support plans to meet the needs of patients.
  • Contribute to the development of the team and the service, identifying areas for improvement and implementing changes.
Requirements
  • Post-qualified experience in Adult Social Work.
  • Registration with Social Work England.
  • Eligibility to work in the UK.
  • Full UK driving license.
  • Excellent communication and interpersonal skills.
  • Ability to work in a fast-paced environment and prioritize tasks effectively.
