Adult Social Worker

Bury Council is seeking a skilled Adult Social Worker to join our Hospital Integrated Discharge Team. As a key member of our team, you will play a vital role in supporting patients to achieve safe and timely discharges from hospital.
Key Responsibilities- Undertake statutory Social Work functions in line with the Care Act and the Mental Capacity Act.
- Work closely with multi-disciplinary teams to facilitate safe and timely discharges of patients from hospital.
- Use a wide variety of personalised support to promote independence, support people with complex needs, and ensure they have the maximum level of choice and control available to them.
- Focus on how people can be supported by their strengths, network of support from family, friends, and local communities.
- Person-centred and compassionate, with excellent listening skills, highly organised, and a team player.
- In-depth understanding of the legal frameworks relevant to Adult Social Work.
- Experience of supporting and mentoring ASYE social workers and students on placement.
- Social Work England registration.
- Eligible to work in the UK.
- Hold a full UK driving licence.
- Substantial post-qualified experience.
