About the Role

We are seeking a highly skilled and dedicated Adult Social Worker to join our Hospital Integrated Discharge Team at Bury Council. As a key member of our team, you will play a vital role in supporting patients to achieve safe and timely discharges from hospital.

Key Responsibilities
  • Undertake statutory Social Work functions in line with the Care Act and the Mental Capacity Act.
  • Work closely with multi-disciplinary teams to facilitate safe and timely discharges of patients from hospital.
  • Provide personalized support to patients, promoting their independence, choice, and control.
  • Collaborate with healthcare professionals to ensure that care and support plans reflect individual needs and strengths.
  • Contribute to the development and implementation of care plans that promote the principles of The Care Act and Mental Capacity Act.
Requirements
  • Person-centred and compassionate approach to social work.
  • Excellent listening skills and ability to work effectively with patients, families, and healthcare professionals.
  • Strong understanding of legal frameworks relevant to Adult Social Work.
  • Experience of supporting and mentoring ASYE social workers and students on placement.
  • Social Work England registration and eligibility to work in the UK.
  • Full UK driving licence.
  • Substantial post-qualified experience in Adult Social Work.
