We are seeking a skilled and experienced Homeless Housing Coordinator to join our team at Brackenberry Ltd. As a key member of our team, you will play a vital role in supporting individuals experiencing homelessness in Enfield.

Key Responsibilities:
  • Deliver a proactive and person-centered approach to case management, ensuring compliance with the Homelessness Reduction Act.
  • Conduct comprehensive assessments to develop focused and co-produced Personal Housing Plans (PHPs) for each applicant.
  • Assess applicants under the prevention and relief duties, issuing S.184 final decision letters and maintaining effective communication with customers.
  • Undertake risk management plans for applicants, utilizing information from various stakeholders and the applicant themselves.
  • Identify, report, and follow up on any safeguarding concerns to minimize risk.
Requirements:
  • High level of education or equivalent demonstrable experience.
  • Evidence of Continuous Professional Development.
  • Significant experience in providing specialist homelessness casework to applicants with complex needs.
  • Experience working collaboratively to improve outcomes for homeless individuals with complex needs.
