Job Summary

An exciting opportunity has arisen in Hywel Dda University Health Board to join our Acute Medical Rehabilitation team. This innovative team is dedicated to supporting the recovery and rehabilitation of patients across medical wards. The post will involve working within Glangwili Hospital's medical wards, including cardiac, respiratory, endocrine, and gastroenterology, as well as frailty and major trauma.

Main Duties

The post holder will work as part of a multi-disciplinary team, including Physiotherapists, Occupational Therapists, Social Workers, Discharge Liaison Nurses, Nurses, and Doctors. Key responsibilities will include:

  • Supporting and completing holistic patient assessments, considering rehabilitation needs and strategies to aid safe and timely discharge.
  • Facilitating continuity of patient care, with a degree of outreach into the community.
  • Delivering rehabilitation to patients admitted to medical wards to promote independence, including bed, chair, and standing activities, gait re-education, and gym-based exercise programs.
  • Supporting patients at times alone, under the guidance of qualified Physiotherapists, with recognition of scope of practice and when qualified support is required.

Hywel Dda University Health Board is the planner and provider of NHS healthcare services for people in Carmarthenshire, Ceredigion, Pembrokeshire, and its bordering counties. Our 11,000 members of staff provide primary, community, in-hospital, mental health, and learning disabilities services for around 384,000 people across a quarter of the landmass of Wales.

We do this in partnership with our three local authorities and public, private, and third sector colleagues, including our volunteers, through:

  • Four main hospitals: Bronglais General in Aberystwyth, Glangwili General in Carmarthen, Prince Philip in Llanelli, and Withybush General in Haverfordwest.
  • Seven community hospitals: Amman Valley and Llandovery in Carmarthenshire; Tregaron, Aberaeron, and Cardigan in Ceredigion; and Tenby and South Pembrokeshire Hospital Health and Social Care Resource Centre in Pembrokeshire.
  • 48 general practices, 47 dental practices, 99 community pharmacies, 44 general ophthalmic practices, and 17 domiciliary only providers and 11 health centres.
  • Numerous locations providing mental health and learning disabilities services.
  • Highly specialised and tertiary services commissioned by the Welsh Health Specialised Services Committee.
Person Specification

The ideal candidate will possess:

  • A good command of the English language and basic maths.
  • NVQ level 3 or equivalent.
  • Relevant additional courses to support clinical work.
  • A significant knowledge and experience of the model of physiotherapy and ability to support therapeutic intervention with arms-length supervision.
  • Broad range of experience as a support worker in therapy services or associated fields.

Desirable qualifications include:

  • IT knowledge of data collection systems.
  • Professional, technical, or academic qualification relating to clinical practice up to 180 credits level 4.

Essential experience includes:

  • Demonstrated manual handling and manipulation skills to help restore patients' functional ability.
  • Demonstrated visual acuity and perceptual skills.
  • Ability to work unsupervised in various settings, taking responsibility for a delegated workload.
  • Working knowledge of specialist therapeutic equipment appropriate to clinical specialty.

Desirable experience includes:

  • Understanding the role of other staff groups in health and social care.
  • Supervision of staff.
  • Additional clinical or healthcare-related experience.
  • Experience of working with groups of patients.

Other essential requirements include:

  • Flexibility to meet the needs of the service and potential need to work on various sites.
  • Ability to travel between sites in a timely manner.

Desirable language skills include:

  • Welsh Speaker (Level 1).
