Clinical Director Opportunity

We are seeking a highly skilled and experienced Clinical Director to lead our community services and drive the transformation of planned and unplanned older people's care within the Newcastle upon Tyne Hospitals NHS Foundation Trust.

The successful candidate will work closely with senior leaders in Older People's Medicine and Community Services to develop and implement patient pathways that improve access to timely community care and reduce hospital admissions.

Key responsibilities will include:

  1. Managing urgent older people's presentations and ensuring patients receive timely and appropriate care.
  2. Developing and implementing clinical strategies to improve patient outcomes and reduce hospital admissions.
  3. Working closely with hospital teams, community services, and primary care providers to develop and implement integrated care pathways.
  4. Providing clinical leadership and expertise to support the development of community services and primary care.
  5. Representing the trust at local and national level to promote the interests of older people's care and community services.

The ideal candidate will have a strong clinical background in older people's medicine, experience of leading clinical services, and a proven track record of improving patient outcomes and reducing hospital admissions.
