Key Responsibilities:

* Develop and review support plans to ensure the individual's life goals and aspirations are at the centre of their support
* Build and maintain positive relationships with the individual and their support network
* Work in partnership with stakeholders to ensure the individual's needs are met
* Enable the individual to take part in their chosen activities and access local community opportunities

Requirements:

* Experience of working within social care is desirable but not essential
* Willingness to learn and develop into the role
* Ability to work flexibly, including weekends and evenings
