Cardiff, Cardiff, United Kingdom Cardiff and Vale University Health Board Full time

Position Overview

An exceptional opportunity has emerged within the Inflammatory Bowel Disease (IBD) Team at Cardiff and Vale University Health Board for Band 6 Clinical Nurse Specialists. This role is pivotal in providing support to individuals affected by IBD.

The service is dedicated to the care of over 4000 patients annually, empowering them to manage their condition effectively, remain at home, and uphold a healthy lifestyle. The team operates a comprehensive outpatient service catering to patients with complex needs, including specialized clinics, advice lines, self-management programs, and biologic therapy in a dedicated infusion suite. Additionally, the IBD service extends its expertise to GP practices and inpatient units.

The successful candidate will work across multiple hospital sites, with a primary focus on delivering care in a collaborative environment.

The IBD Service seeks a seasoned IBD Clinical Nurse Specialist or an individual with relevant transferable skills to enhance the service's quality and effectiveness. While a Non-Medical Prescribing qualification is advantageous, support will be provided to obtain this qualification if necessary.

We are in search of a proactive, compassionate, and adaptable individual eager to advance the IBD Service alongside the Lead Consultant and fellow nursing professionals.

Key Responsibilities

  • Deliver expert nursing knowledge, support, and guidance for patients with IBD, focusing on the assessment, development, implementation, and evaluation of care plans.
  • Engage in complex decision-making and provide counseling for patients and their families.
  • Coordinate and manage patient self-directed care, including follow-up and monitoring of patients at elevated risk for colon cancer and those susceptible to osteoporosis due to steroid treatment.
  • Provide training and oversight for patients receiving Methotrexate/biologic injections, ensuring appropriate follow-up care.
  • Organize and manage nurse-led outpatient clinics, independently assessing newly diagnosed IBD patients, conducting clinical evaluations, and formulating intricate management strategies with medical team support.
  • Utilize advanced nursing skills as required, including cannulation, venepuncture, and radiological requests, in accordance with organizational policies (additional educational support will be available).
  • Oversee patient management from diagnosis through ongoing investigations and treatment adjustments, making informed decisions and consulting with senior colleagues or consultants as necessary.
  • Contribute to audit and research initiatives within the service.

The ability to communicate in Welsh is desirable for this position; applications are welcome from both Welsh and English speakers.
