Job Title: Band 5 Children's Community Nurse

We are seeking a highly motivated and experienced Band 5 Children's Community Nurse to join our Paediatric Respiratory Team at Milton Keynes University Hospital NHS Foundation Trust.

About the Role

This is an exciting opportunity to work in a dynamic and supportive team, providing high-quality care to children and young people with respiratory conditions. As a Band 5 Children's Community Nurse, you will be responsible for assessing, planning, implementing, and evaluating care for these patients, working closely with the multi-disciplinary team to ensure seamless care transitions from hospital to home.

Key Responsibilities
  • Provide expert care and support to children and young people with respiratory conditions, including asthma and other chronic respiratory diseases.
  • Work collaboratively with the Paediatric Respiratory Team, Ward 4, Ward 5, PED, and the Children's Primary Care Team to ensure comprehensive care and support.
  • Assess, plan, implement, and evaluate care for patients, including developing and implementing individualized care plans.
  • Communicate effectively with patients, families, and healthcare professionals to ensure seamless care transitions and optimal patient outcomes.
  • Participate in education and training programs to enhance knowledge and skills in paediatric respiratory care.
Requirements
  • Registered Children's Nurse with a valid NMC registration.
  • Experience working in paediatric respiratory care, preferably in a community setting.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
  • Ability to work autonomously and as part of a team, with a flexible and adaptable approach to care delivery.
  • Knowledge of national guidelines and best practices in paediatric respiratory care.
