CNWL NHS Foundation Trust is seeking a dedicated and skilled Tissue Viability Nurse to enhance our community nursing team within the Tissue Viability Service.
The ideal candidate will be a registered adult nurse, capable of functioning independently while adhering to established protocols and organizational policies.
Key Responsibilities:
- Provide expert clinical support in managing patients with complex and chronic wounds, including pressure ulcers and leg ulcers.
- Collaborate with community teams, inpatient units, and primary health care staff to deliver high-quality wound care.
- Assist in the development and implementation of educational programs for healthcare personnel focused on wound and skin care.
- Monitor and report on pressure ulcer prevalence and incidence, contributing to investigations as necessary.
- Support the management of pressure ulcer prevention equipment and offer specialist advice to prescribers.
Clinical Duties:
- Conduct evidence-based assessments and treatment plans for complex tissue viability cases.
- Document clinical care plans and maintain high standards of record-keeping.
- Engage with multidisciplinary teams to ensure comprehensive patient care.
- Participate in complex wound clinics and develop advanced skills in holistic assessments and bandaging techniques.
