Job Summary

We are seeking a highly motivated and organized Care Coordinator to join our team. The successful candidate will be responsible for coordinating service delivery, managing inquiries, and undertaking formal checks on staff. They will also be responsible for creating and updating client care plans, and contributing to the delivery of a high standard of care through effective communication and teamwork.

Key Responsibilities
  • Assist with the creation of rotas and working patterns for all staff to ensure all service is covered.
  • Coordinate service delivery daily, including managing inquiries from staff, clients, and other health professionals.
  • Undertake formal checks on staff in the field.
  • Undertake initial care assessments and create and update client care plans.
  • Contribute to the delivery of a high standard of care through effective communication and teamwork.
  • Provide cover delivering care in contingency.
Requirements
  • Must have a full UK driving license and car.
  • Must have a smartphone.
  • Have a natural caring approach to life.
  • Excellent communication and teamwork skills.
  • Experience in a care setting is preferred.
