The Clinical Nurse Specialist will establish processes for the efficient management of patients with chronic conditions, utilizing evidence-based methodologies, particularly for elderly and homebound individuals.
As a key member of the multidisciplinary healthcare team, the incumbent will ensure that nursing services are effectively provided to the designated patient demographic.
Ormskirk House Surgery is committed to ensuring that the incumbent has access to suitable clinical supervision and a designated individual within the organization to offer general guidance and support on a daily basis.
