El equipo de Salud Mental Comunitaria está en búsqueda de un Consultor que brinde apoyo médico al Servicio de Memoria y al equipo de Enlace con Residencias de Ancianos. El Consultor colaborará dentro de tres equipos dedicados a áreas específicas en la región del Grupo de Comisionamiento Clínico.
El Consultor será responsable de proporcionar intervenciones directas a los usuarios de servicios con necesidades de salud mental y física, incluyendo aquellos difíciles de involucrar. La asistencia regular a reuniones de Equipos Multidisciplinarios, la evaluación y tratamiento de pacientes ambulatorios, así como la realización de reuniones de CPA son responsabilidades clave de este rol.
- Proporcionar asesoramiento a los miembros del equipo
- Asistencia a reuniones de MDT
- Evaluación y tratamiento en clínicas
- Reuniones de CPA para pacientes ambulatorios
- Visitas a pacientes en su lugar de residencia
- Colaborar con otras áreas de servicio y profesionales médicos
- Preparar informes para Tribunales de Salud Mental
- Participar en reuniones de Tribunales de Salud Mental
- Proporcionar responsabilidad médica y AC para prácticas de GP designadas
El candidato seleccionado tendrá la oportunidad de trabajar con un equipo dedicado de profesionales de la salud y contribuir al bienestar de los adultos mayores en la comunidad. Se requieren habilidades de liderazgo, comunicación efectiva y la capacidad de gestionar el trabajo clínico para este rol.
