About the Role:

We are seeking a qualified Advanced Practitioner to join our Primary Care Network Community Beds team at Sandwell and West Birmingham NHS Trust.

Key Responsibilities:

  • Provide clinical expertise and leadership in primary care, working closely with the PCN Lead ACP and wider team to improve the quality of care provided.
  • Collaborate with multidisciplinary teams, including GPs, Nurses, Clinical Pharmacists, Allied Health Professionals, and Administrators, to deliver high-quality patient care.
