Lead Care Facilitator

Position Overview
As a Lead Care Facilitator, your primary responsibility will be to engage with the community, conducting evaluations and overseeing the performance of our Care Assistants. You will be responsible for mentoring, guiding, and providing daily support to a dedicated team of caregivers, collaborating closely with our Registered Manager to ensure the effective operation of the team. This role will also require participation in an on-call rota to address urgent issues related to service continuity.
Weekend and out-of-hours work will be compensated with enhanced pay rates, as will involvement in the on-call service.

- Delivering personal care services,

- Participating in the on-call service outside of standard office hours.
A valid UK driving licence is required, and you will be working in clients' residences as well as at our office.

Job Duties
All assigned tasks will be performed in accordance with established protocols developed by professionals within the team. You will assist individuals with a wide range of personal care tasks, such as promoting independence in washing and dressing, encouraging proper nutrition and medication management, and conducting health or therapeutic observations under the guidance of a Registered Practitioner.
This role also provides opportunities to enhance clinical skills, including wound care, catheter management, stoma care, and PEG feeding assistance, as well as competencies related to Physiotherapy or Occupational Therapy, such as supporting therapy plans and assessing equipment needs under the supervision of the relevant Registered Practitioner.
You will collaborate with all mainstream health and care services to ensure that individuals' needs are met efficiently and cohesively, advocating for them to minimize the need for repetitive information sharing. You will conduct appropriate risk assessments and contribute to professional evaluations regarding both health and care requirements, liaising with relevant professionals to maintain and update pertinent health and care records.
This role will assist the Registered Practitioner in delivering routine and essential health and social care support to individuals. The post holder will respond to clients' personal health (both physical and mental) and social care needs, as well as broader independent living skills, such as addressing social isolation and employment needs, adapting to the individual's presenting requirements.
Responsibilities Include:
- Assisting individuals in maintaining a safe and healthy living environment,
- Supporting nutritional and hydration needs, including clinical tasks such as PEG feeding and stoma care,
- Helping individuals maintain personal appearance and self-esteem through personal care,
- Facilitating rest and sleep by addressing comfort needs,
- Applying a person-centered approach to deliver routine and essential health and care services,
- Ensuring the health and well-being of individuals in care, promptly reporting any concerns,
- Administering medications in accordance with established policies and guidelines,
- Encouraging access to universal services and community resources, including voluntary sector services,
- Maximizing individuals' ability to manage their own resources and circumstances, promoting independence while managing risks positively,
- Supporting individuals with long-term health conditions, seeking professional advice as necessary to promote well-being,
- Contributing to person-centered support plans and reviews to achieve identified health and social outcomes,
- Conducting ongoing monitoring throughout the service duration, ensuring accurate recording and reporting,
- Assisting adults in maintaining employment by providing workplace support and promoting natural workplace supports where possible,
- Delivering personal care in a manner that upholds dignity and independence,
- Providing services to support wound care, monitoring and ordering supplies as needed, and reporting skin integrity issues to professionals,
- Assessing and, in some cases, delivering equipment and aids to support independent living,
- Taking responsibility as a case holder for equipment, adaptations, or telecare provided following assessment, or serving as an additional assessor for a caseload where other services are also provided,
- Identifying, fitting, and demonstrating less complex equipment, adaptations, and telecare to enhance independence.
