Clinical Lead for Early Intervention Team

We are seeking an experienced and skilled professional to join our Early Intervention Team as a Clinical Lead. The successful candidate will be responsible for leading the team in providing proactive admission avoidance services, early discharges, complex discharge planning, frailty screening, and therapy assessments for patients admitted to the Emergency Department, Assessment Units, Frailty Unit, and in-patient areas.
Key Responsibilities- Lead the team in providing advanced assessment, treatment, and clinical management of patients attending the Emergency Department.
- Complete transdisciplinary assessments, management plans, and recommendations for patients in the Non DME wards.
- Support the other EIT Clinical Leads in providing training and support for EIT Staff, including Frailty Specialist nurses and CDSNs, and Physiotherapists/Occupational Therapists in the team.
- Develop efficient and effective working processes with acute inpatient colleagues and our community and primary care partners to prevent unwanted admission or improve flow/reduce length of stay for patients who are admitted.
- Work with the service Lead and other Band 7 clinical lead Nurses and Therapists to develop the service to best meet changing health and social care agendas.
- Capture data and report Key Performance Indicators for the service.
- Identify and lead on quality improvement projects evaluating the service against the service objectives.
- Be responsible for all aspects of staff line management and recruitment in line with HR policy.

Person SpecificationWe are looking for a candidate with the following qualifications and experience:
- Registered Nurse / Registered Physiotherapist or Occupational Therapist / Registered Practitioner
- Evidence of continuous professional development
- Post registration qualification in speciality or willingness to complete
Desirable qualifications include a degree and research module. Essential experience includes:
- Demonstrate expert skills from a broad range of clinical environments
- Previous experience in care of older adults
- Accept personal accountability for own practice
- Experience of formal/informal teaching
- Involvement in service development
- Competency and experience in working within a transdisciplinary environment
- Experience leading audit, demonstrable involvement in identifying and participating in quality improvement projects and initiating change
- Experience supervising and appraising staff and students
Desirable experience includes community experience and recent experience working at senior level (band 6 or 7). Essential knowledge includes:
- Understanding of evidence-based practice
- Understanding of Clinical Governance
- Understanding and Knowledge of Frailty Syndromes and the impact they may have on a person living with frailty
- Good knowledge of current best practice in Frailty within emergency care settings and in patient areas
Essential skills include:
- Good verbal and written communication skills
- Capable of working as an integral member of a small team
- Adaptable to changing situations due to both workload and colleagues' needs
- Ability to undertake teaching of staff
- Ability to work unsupervised and manage own workload effectively with time management
- Be able to demonstrate flexibility and reliability
Desirable skills include assertiveness/perceptive counselling skills, documentation, audit, and presentation skills, leadership skills, and advanced communication skills.
