Early Intervention Team Specialist

We are seeking a highly skilled and experienced professional to join our Early Intervention Team as a Proactive Discharge Planning and Frailty Specialist. As a key member of our team, you will play a vital role in providing expert care to patients attending the Emergency Department and in-patient areas.
Key Responsibilities- Conduct advanced assessments and develop comprehensive management plans for patients with complex needs.
- Lead on quality improvement projects to evaluate the service against its objectives and identify areas for improvement.
- Collaborate with other clinical leads to develop and implement efficient working processes with acute inpatient colleagues and community partners.
- Supervise and appraise staff, and provide training and support to ensure high-quality care.
- Develop and maintain strong relationships with stakeholders, including patients, families, and healthcare professionals.
- Contribute to the development of the service, ensuring it meets the changing needs of patients and the healthcare system.
