About the Role

Greensand Surgery is excited to announce an opportunity for a skilled Chronic Conditions Specialist Nurse to join our dedicated team on a part-time basis, working 18 hours per week. In this pivotal role, you will uphold a high standard of chronic disease management, focusing on the diagnosis, monitoring, and development of personalized management plans in collaboration with patients and other healthcare professionals.

Your expertise will primarily be in managing conditions such as Diabetes, Chronic Heart Disease (CHD), and Heart Failure (HF), adhering strictly to established practice guidelines and protocols. You will work closely with a multi-disciplinary general practice team to address patient needs, conduct assessments, and review healthcare requirements, all while enhancing well-being, self-empowerment, and quality of life.

Main Responsibilities:

  • Oversee the management of long-term condition clinics focusing on diabetes, chronic heart disease, and heart failure.
  • Identify health promotion opportunities and provide concise lifestyle advice utilizing motivational interviewing techniques, including alcohol screening and smoking cessation support.
  • Conduct annual reviews for patients with long-term conditions.
  • Perform medication reviews and make necessary adjustments based on patient results.
  • Assess patients presenting for acute services on the same day.
  • Manage pathology, workflow, and associated tasks efficiently.
  • Proactively enhance and influence the patient care pathway.

About the Candidate

We welcome applications from individuals who possess the following qualifications, registrations, skills, and experience:

Essential:

  • Registered Nurse.
  • NMC 1st level registration.
  • Accredited diploma (or higher) in at least one chronic disease area, such as diabetes, CHD, or HF.
  • Demonstrated commitment to continued professional development.
  • Experience in managing long-term conditions, specifically CHD, HF, and Diabetes.
  • Background in primary or community nursing care.
  • Understanding of QOF and enhanced service management.
  • Willingness to pursue further training and education.
  • Desire to improve and develop the patient experience and care pathway.
  • Proficiency in Microsoft software.

Desirable:

  • Non-medical prescribing qualification (or willingness to pursue).
  • Experience with SystmOne and Arden's templates.
  • Involvement in the implementation and utilization of clinical protocols and guidelines.
