About The Role

The Hounslow Community Recovery Service is an integrated health and social care initiative aimed at supporting individuals' independence following health challenges or life changes.

This team focuses on community rehabilitation and reablement, working to restore and enhance independence while preventing further decline in physical, psychological, and social well-being.

Our integrated team comprises various health and social care professionals, all equipped with essential skills to deliver cohesive support.

Team members will also possess specialized knowledge to assist with specific clinical pathways, ensuring comprehensive care.

Key Responsibilities
  • Manage your own community rehabilitation caseload independently.
  • Participate in 7-day working rotas as necessary, including emergency cover.
  • Conduct thorough Occupational Therapy assessments for residents with diverse or complex needs.
  • Identify and act on safeguarding concerns for vulnerable residents promptly.
  • Maintain professional accountability for all aspects of your work, including clinical risk management.
Your Skills
  • Deliver high-quality occupational therapy services autonomously and collaboratively within a multidisciplinary team.
  • Effectively negotiate with residents and their families to meet their needs within available resources.
  • Utilize various communication techniques to facilitate rehabilitation and treatment.
  • Adhere to Royal College of Occupational Therapy guidelines and maintain accurate records while monitoring service quality.
Essential Qualifications
  • Diploma in Occupational Therapy (state registered) or Degree in Occupational Therapy.
  • Experience in a rehabilitation setting is required.
  • An Enhanced DBS check is mandatory for this role.

This position is considered customer-facing, necessitating a proficient command of spoken English for effective job performance.
