Support Worker for Older Adults with Mental Health Needs

We are a dynamic and compassionate Community Mental Health Team working with Older Adults in North Manchester. Our client group (over 65 years) either have a functional or an organic diagnosis. We are seeking a part-time Support Worker to join our friendly team. The post is for hours a week.
Main Responsibilities- Work as part of the multi-disciplinary team to provide support, advice, and practical assistance to an allocated group of service users under the direction and supervision of a qualified member of staff.
- Support in delivering a care plan as agreed with the service user and/or their carers where appropriate.
- Report back to the qualified member of staff with any concerns or safeguarding issues.
- Provide practical support including wellbeing checks as needed, involving travelling across the North Manchester area and seeing people in their own homes.
- Participate in Support Worker Duty Rota and attend daily Zoning meetings.
- NVQ Level 2 or equivalent experience demonstrated.
- An ability to work independently and within a team.
- Awareness of mental health issues.
