We are seeking a proactive, versatile, and innovative individual to become a vital member of our team, contributing to the advancement of this groundbreaking service. Virtual Wards are designed to avert hospital admissions or facilitate earlier supported discharges, allowing patients to recuperate from their illnesses in the comfort of their homes while receiving acute care, treatments, and remote monitoring.

This service also enhances hospital bed capacity, ensuring that more patients can access timely medical attention.

As part of the integrated virtual ward team, you will collaborate across both Acute and Community settings to deliver exceptional patient care, clinical governance, and support to the entire multidisciplinary team (MDT).

You will work alongside a skilled group of Consultants, Advanced Clinical Practitioners, Clinical Team Leaders, and dedicated clinical and non-clinical staff to ensure outstanding patient care throughout Central Lancashire.

This position is ideal for individuals eager to engage in a novel and innovative service, aiding in the expansion and development of new pathways within this integrated Virtual Ward service.

The Virtual Ward operates seven days a week from 8 am to 8 pm, and the successful candidate will be expected to work on a rotational basis to meet service requirements.

Main Responsibilities

The successful candidate will provide essential administrative support and effective coordination of Virtual Ward activities, ensuring that all information remains secure and confidentiality is upheld at all times.

This role necessitates excellent communication skills, as you will interact with patients, including those who may be distressed or anxious, treating them with empathy and professionalism. It is crucial to maintain the Trust's professional image at all times.

You will be in close contact with clinical teams, operational staff, workforce, financial colleagues, and other support services, acting as a liaison to facilitate patient care throughout their virtual ward experience.

As this is a developing service, the candidate must be proactive, adept at problem-solving, and able to use their initiative to swiftly address issues. There is potential for growth as the virtual ward service expands its capacity and scope.

Detailed Responsibilities

- Support the patient journey by serving as a point of contact and ensuring timely responses to patient inquiries.

- Escalate patient alerts promptly using the Docobo remote monitoring platform.

- Maintain a high-quality customer service function, coordinating ongoing communication with referrers and relevant departments such as radiology and pathology.
