We are seeking a highly skilled and experienced Complex Care Sister to join our Continuing Health Care Team in Swansea Bay University Health Board.
Key Responsibilities- Manage a team of Health Care Support Workers to provide high-quality care to patients with complex needs.
- Develop and implement care plans in collaboration with patients, families, and other healthcare professionals.
- Conduct clinical reviews to ensure patients receive the appropriate level of care and make referrals to alternative care agencies as needed.
- Provide operational support to healthcare workers, including sickness reviews and PADR.
- Monitor and improve community care packages, including auditing and preventing unnecessary hospital admissions.
- Work collaboratively with quality outcomes of the QCF framework to support healthcare workers through education and quality assurance.

Requirements- First-level registered nurse with post-registration experience in managing patients with complex care needs.
- Experience of team management, community nursing, and working with patients with complex health needs.
- Knowledge of quality improvement, teaching, and caseload management.
- Ability to work collaboratively with other agencies and services.
