About the Role

We are seeking a highly motivated and compassionate Community Health Assistant to join our team at HCRG Care Group. As a Community Health Assistant, you will play a vital role in providing rehabilitation and care services to patients in their own homes.

Key Responsibilities
  • Assist in providing rehabilitation and care services to patients in their own homes.
  • Support patients with home exercise programs as prescribed by the therapy team.
  • Maintain knowledge, skills, and attitudes necessary to deliver a high-quality service responsive to the changing needs of patients.
  • Provide clerical and administrative support to the team.
  • Deliver care in accordance with organizational policies, procedures, and guidelines, according to competencies, as delegated by the Operational Lead.
  • Report any changes in patient condition to the Operational Lead.
