Position OverviewWe are seeking a Qualified Social Worker to join our Referral and Assessment team within the social care sector. This role is essential in providing comprehensive support to vulnerable populations.
Key Responsibilities- Participate in the duty rota to ensure continuous support.
- Initiate care proceedings as necessary.
- Collaborate with various agencies to assist families and children.
- Prepare initial child protection reports and conference documentation.
The ideal candidate will possess:
- A recognized social work qualification (degree or diploma).
- Registration with the relevant regulatory body.
- Demonstrated experience in a social work capacity, preferably within Referral and Assessment.
- A solid understanding of applicable legislation, policies, and procedures in social work.
As part of our team, you will:
- Provide information, guidance, and referrals to vulnerable adults and their caregivers.
- Manage referrals for Adult Social Care Services, prioritizing and assessing risk daily.
- Utilize your expertise to determine eligibility for Community Care Services.
- Facilitate interim Adult Social Care Services to ensure the safety and well-being of residents and their caregivers.
- Initiate Self Directed Support to establish a planning amount for Personal Budgets.
