Job Overview

We are seeking a highly motivated and experienced Community Health Nurse to join our team in Havering. The successful candidate will work as a member of our integrated community health care team, providing holistic healthcare to patients in their own homes, residential care homes, and nurse-led clinics.

Main Responsibilities

  • Provide high-quality patient care, working autonomously and as part of a team, to deliver individualized care plans.
  • Liaise with GPs, other professionals, and statutory and voluntary agencies to ensure seamless care delivery.
  • Work with patients and carers as partners in care, promoting patient-centered approaches.
  • Initiate patient assessment and care planning with the support of a senior colleague.
  • Develop and maintain effective relationships with patients, families, and carers, ensuring excellent communication and interpersonal skills.

Requirements

  • NMC Registration and current NMC R/N or any other 1st-level Nursing, RN12 Adult qualification.
  • Experience of working with professionals from other disciplines, as a member of a team, and autonomously.
  • Ability to work with minimal supervision, prioritize tasks, and make decisions.
  • Excellent communication, interpersonal, and problem-solving skills.
  • Valid UK driving license and access to a car for work purposes.
