Job Title: Community Heart Failure Nurse Specialist

We are seeking an experienced and skilled Community Heart Failure Nurse Specialist to join our team at Midlands Partnership NHS Foundation Trust.

As a Community Heart Failure Nurse Specialist, you will be responsible for assessing, planning, and delivering evidence-based care to patients in their own homes.

You will work collaboratively with patients, families, and healthcare professionals to develop and implement individualized care plans, promoting independence, self-management, and improved health outcomes.

Key Responsibilities
  • Assess and manage patients with heart failure, including medication management and symptom control
  • Develop and implement individualized care plans, taking into account patient preferences and needs
  • Provide education and support to patients and their families, promoting self-management and independence
  • Collaborate with healthcare professionals to ensure seamless care transitions and coordination
  • Contribute to the development and implementation of service improvement initiatives
Person Specification
  • Registered Nurse with current NMC registration
  • Postgraduate studies in a relevant field, such as heart failure or cardiology
  • Experience in community nursing, with a focus on heart failure care
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals
  • Ability to work autonomously and as part of a team, with a flexible approach to work
