About the Role

We are seeking a highly motivated and experienced Support Coordinator to join our team in North Tyneside. As a Support Coordinator, you will play a vital role in supporting our customers with complex needs to develop their skills and live more independently in the community.

Key Responsibilities

  • Creating personalized support plans with customers and working closely with social workers, community treatment teams, and other stakeholders to ensure comprehensive support.
  • Enabling customers to access community resources, prompting with medication, supporting with finances, and assisting with daily living activities.
  • Providing emotional support and guidance to customers, promoting their independence and well-being.
  • Conducting risk assessments, support planning, and regular reviews to ensure customers' needs are met.
  • Collaborating with colleagues to provide a holistic approach to customer support.

Requirements

  • Experience in creating person-centered support plans and supporting colleagues in working to these plans.
  • Ability to work independently, remain calm under pressure, and have a resilient approach.
  • Valid driving license and access to a vehicle insured for business purposes.
  • Enhanced DBS check.
