Responsibilities- Provide comprehensive support to residents navigating complex social, housing, and personal challenges.
- Collaborate with community partners, including probation services, mental health professionals, and local organizations, to ensure residents receive holistic care.
- Facilitate access to essential services such as healthcare, employment assistance, legal aid, and financial counseling.
- Develop individualized support plans tailored to the unique needs of each resident.
- Monitor residents' progress, provide ongoing guidance, and celebrate their achievements.
- Maintain accurate and confidential records in accordance with organizational policies and procedures.
