Community Support Specialist

We are seeking a compassionate and dedicated Community Support Specialist to join our team. In this role, you will play a vital part in assisting residents with diverse needs, connecting them with essential resources, and advocating for their well-being.

Responsibilities
  • Provide comprehensive support to residents navigating complex social, housing, and personal challenges.
  • Collaborate with community partners, including probation services, mental health professionals, and local organizations, to ensure residents receive holistic care.
  • Facilitate access to essential services such as healthcare, employment assistance, legal aid, and financial counseling.
  • Develop individualized support plans tailored to the unique needs of each resident.
  • Monitor residents' progress, provide ongoing guidance, and celebrate their achievements.
  • Maintain accurate and confidential records in accordance with organizational policies and procedures.
