About the Role

We are seeking a dynamic and passionate individual to join our team as a Team Facilitator. As a key member of our management team, you will play a crucial role in supporting our frontline staff to deliver high-quality, bespoke care to the people we support.

Key Responsibilities
  • Facilitate person-centred planning techniques to deliver excellent support practices.
  • Lead and support frontline staff to take ownership and responsibility for the quality of support they deliver.
  • Effectively communicate with the people we support, staff teams, families, and external care professionals.
  • Create and deliver robust service designs, support strategies, and risk assessments while managing individual budgets.
  • Ensure excellent organisational skills and the ability to prioritise your workload while working under pressure.
