About the Role

We are seeking a highly motivated and compassionate individual to join our Community Team as a Care and Community Services Officer. As a key member of our team, you will play a vital role in providing mental health care and support to older adults in the community.

Main Responsibilities
  • Assist in the implementation of care plans to ensure service users' rights to privacy, dignity, choice, independence, and fulfillment are upheld.
  • Participate in multi-professional assessments and reporting back to other care agencies.
  • Provide support to service users and/or carers in accordance with care plans, maintaining social networks, ensuring a safe environment, and facilitating communication with other service providers.
  • Participate in multi-professional reviews and Care Programme Approach/Care Management meetings.
  • Report any changes in circumstances regarding service users' care to relevant agencies, improving the quality of life, rehabilitation, and independence of service users and their carers.
  • Ensure that all care services provided are offered without prejudice, regardless of age, creed, gender, and ethnicity, within your sphere of responsibility.
  • Develop awareness of service users' cultural and ethnic needs to facilitate appropriate support and care.
Requirements
  • Good standard of education (to include Maths and English GCSE grade C or above - or equivalent).
  • Computer literate (able to use Word, Excel, PowerPoint).
  • Knowledge of legislation related to mental health and social care (e.g. Mental Health Act, Mental Capacity Act, National Service Framework, National Dementia Strategy, Care Programme Approach, etc).
  • NVQ in Health and Social Care.
  • Experience of working with people with dementia.
What We Offer

We offer a competitive salary of £24,071 to £25,674 per annum, pro rata, plus 5% of basic salary (min £1,258 max £2,122).
