Position Overview

An exceptional opportunity has arisen to deliver nursing services to individuals within the community setting while overseeing the responsibilities of junior staff and assisting the team leader.

This role involves working under the guidance of the Integrated Team leader, focusing on the evaluation and management of the health requirements of a designated population, including end-of-life care and patients with chronic conditions or intricate nursing needs.

Key Responsibilities

The successful candidate will:

  • Deliver nursing services to patients in the community and organize the workload for junior staff while supporting the team leader.
  • Engage in the provision of nursing care and assistance to clients in the community, demonstrating the necessary confidence and skills to operate independently.
  • Collaborate with the Integrated Team leader to assess and manage the health needs of a specified population, including end-of-life care and patients with chronic or complex nursing requirements.
  • Support and, where feasible, enable patients to remain in their preferred care settings, promoting self-management to enhance their independence and quality of life throughout the week.
  • Maintain effective communication with all agencies and professionals involved in client care, ensuring that clients and their families are included in discussions and decision-making processes.
  • Act as the Key worker, coordinating care for patients requiring management of Long-Term Conditions or End-of-Life Care at home.
  • Plan duty schedules to ensure adequate coverage for client safety and execute necessary nursing and care procedures.

Essential Qualifications

The position is open to individuals who possess a current NMC PIN to practice as a Registered Nurse or Midwife in the UK, or those who are enrolled in a program that will lead to obtaining their full NMC PIN within the next three months.
