We are seeking a highly motivated and experienced Social Worker to join our Adult Mental Health Services team in the Vale of Glamorgan Council. The successful candidate will work closely with citizens who experience severe and enduring mental health difficulties, providing assessments, care plans, and support to promote independence and resilience.
Key Responsibilities- Conduct comprehensive assessments of adult service users and carers to identify needs and develop care plans.
- Work in partnership with statutory and voluntary organisations to promote the independence of service users.
- Provide supervision, coaching, and mentoring to colleagues.
- Assist the Local Authority in discharging its statutory duties under the Mental Health Act.
- Experience of working with adults who are vulnerable through illness, disability, or age-related frailty.
- Assessment and care planning skills.
- Ability to work autonomously and as part of a multi-disciplinary team.
- Competence in producing reports and risk assessments.
