About the Role

We are seeking a highly motivated and compassionate Health and Social Care Support Worker to join our team at Livewell Southwest. As a key member of our team, you will play a vital role in providing holistic care to patients, promoting independence, and supporting their rehabilitation journey.

Key Responsibilities
  • Provide high-quality care to patients, including support with activities of daily living, medication administration, and risk management.
  • Work collaboratively with the multidisciplinary team to develop and implement individualized care plans.
  • Engage patients in activities that promote independence, rehabilitation, and reablement.
  • Maintain accurate and confidential records, adhering to Livewell Southwest's policies and procedures.
  • Participate in ongoing training and development opportunities to enhance your skills and knowledge.
Requirements
  • Vocational qualification at level III or equivalent, with a strong understanding of health and social care principles.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and colleagues.
  • Ability to work independently and as part of a team, with a strong commitment to patient-centered care.
  • Knowledge of Livewell Southwest's policies, procedures, and practices, with a focus on confidentiality, data protection, and health and safety.
