Job Overview

An exciting opportunity has arisen within the North West Borough Healthcare NHS Foundation Trust for a Community Learning Disability Health Care Support Worker to support the delivery of specialist health support which promotes the independence, community inclusion and positive reputation of people with learning disabilities.

Main Duties
  • Support the delivery of care plans and programmes by implementing programmes and reporting outcomes.
  • Contribute to the recording of plans and programmes.
  • Act as a nominated worker for individual clients who have learning disabilities and may also display behaviours of concern including physical and verbal aggression, changes in mental health, complex physical health needs.
  • Facilitate and support the development of appropriate personal care and independence with activities of daily living.
  • Support self-help, communication and social skills acquisition.
  • Participate in the implementation of specific programmes of care or therapy to develop and enhance socially appropriate adaptive behaviours under the supervision of qualified staff.
  • Monitor mental and physical health and report outcomes to the qualified practitioner.
  • Provide information to enable the effectiveness of care to be evaluated.
  • Ensure appropriate records are maintained under the direction of the qualified practitioner.

Person Specification
  • NVQ 3 or equivalent level of experience within a care setting.
  • Experience of working with service users with Learning Disabilities and complex needs.
  • Knowledge of Recovery model.
  • An understanding of the organisation of nursing care.
