About the Role

We are seeking an experienced and compassionate Registered Nurse to join our Community Palliative Care Team as a Band 6 Community General Nurse. The successful candidate will provide holistic care to patients with life-limiting conditions, working autonomously and in collaboration with other healthcare professionals to deliver seamless care.

Key Responsibilities
  • Provide palliative care to patients in the community, working autonomously and in collaboration with other healthcare professionals.
  • Liaise with other healthcare agencies to ensure seamless care and support for patients.
  • Develop and implement advanced care plans to reduce inappropriate admissions to secondary care.
  • Work closely with the Palliative Care Team to deliver high-quality end-of-life care.
Person Specification

We are looking for a motivated and experienced Registered Nurse with a minimum of 18 months post-registration experience. The ideal candidate will have experience working with palliative care patients, excellent communication skills, and the ability to work autonomously.
