About the Role:

Elysium Healthcare is seeking an experienced Social Worker to join our multidisciplinary team at St Mary's Hospital in Warrington. As a Social Worker, you will play a vital role in providing care and support to individuals with Acquired Brain Injuries (ABI), progressive neurological conditions, Autism Spectrum Conditions (ASC), and Deaf Mental Health problems.

Key Responsibilities:

  • Manage a complex caseload and conduct social care assessments, risk assessments, and facilitate group activities.
  • Contribute to Interdisciplinary Care (ICR) meetings and support service users in making viable plans for their future.
  • Report to the Safeguarding Lead and manage internal investigations, liaising with local authorities as needed.
  • Participate in 1:1 supervision and complete annual social care reassessments within the CPA format.
  • Facilitate access to welfare rights, housing, advocacy services, and associated support services.

Requirements:

  • Social Work England registration.
  • Social Work degree.
  • Experience working in a similar environment.
