Job Title: Adult Mental Health Practitioner

We are seeking a highly skilled and compassionate Adult Mental Health Practitioner to join our dynamic Community Mental Health Team. As a key member of our team, you will provide high-quality care to older adults with dementia and functional mental health conditions.

About the Role:
  • Provide comprehensive assessments and develop personalized treatment plans for older adults with complex mental health needs.
  • Work collaboratively with a well-established team to deliver high-quality care and support to patients and their families.
  • Conduct regular reviews and assessments to ensure the best possible outcomes for service users.
  • Develop and maintain strong relationships with clients, carers, GPs, and other partners to ensure seamless care coordination.
  • Contribute to the day-to-day operational functions of the team, including completing new assessments and covering office duties.
