About the Role

We are seeking a highly skilled Mental Health Practitioner to join our Complex Needs Service in Macclesfield. As a key member of our team, you will play a vital role in improving outcomes for individuals with complex mental health needs.

Key Responsibilities
  • Conduct individual service user focused specialist assessments to identify needs and develop personalized care plans.
  • Deliver evidence-based interventions to promote recovery and stability, reducing levels of service utilization and improving mental health outcomes.
  • Collaborate with commissioners and care providers to ensure seamless care coordination and effective service delivery.
  • Develop and implement specialist programs of care, care packages, and provide specialized advice in relation to care.

Requirements
  • Minimum of 2 years' experience working with adults with mental health problems, with a complex nature.
  • Proven experience in planning and organizing complex activities or programs, requiring formulation and adjustment.
  • Knowledge of delivering individual service user focused specialist assessments and evidence-based interventions.
