Job Overview

Attention Nurses and Allied Health Professionals with a focus on Lymphoedema

An exceptional opportunity is available to pursue a Lymphoedema qualification while engaging in clinical practice, aimed at evolving into the role of a Specialist Lymphoedema Practitioner through a fully funded program.

The East Lancashire Lymphoedema service encompasses the areas of Burnley, Pendle, Rossendale, Hyndburn, Ribblesdale, Blackburn, and Darwen.

Clinics are situated within health centers, and we provide home visits for patients who are housebound or require palliative care.

A valid driving license and access to a vehicle for work purposes are essential for this position.

We deliver a variety of treatments utilizing the latest technologies and equipment.

Please be aware that continued employment is contingent upon the successful completion of the development program, which is expected to last between 18 to 24 months.

Options for full-time or part-time employment are available based on the applicant's preference.

Key Responsibilities

Assist the Clinical Team Leader in delivering care to patients with long-term conditions, specifically Lymphoedema.

Provide individualized, patient-centered care while empowering patients in their treatment journey.

Participate in educational initiatives for both patients and staff.

Deliver high-quality holistic assessments and care planning, adhering to best practice guidelines in Lymphoedema management.

The ELHT Lymphoedema service has been operational since 2008, comprising a small team with extensive specialist knowledge in Lymphoedema.

We manage both uncomplicated and complex Lymphoedema cases, which allows for diverse educational opportunities with patients and colleagues, and we are passionate about innovative approaches to care.

Our working hours are Monday to Friday afternoons, excluding bank holidays.

Flexible working arrangements may be considered following discussions.

Detailed Responsibilities

Assess, plan, implement, and evaluate care plans, actively seeking evidence-based practices that demonstrate improvements in patient care.

Triage referrals from various primary and secondary healthcare professionals.

Collaborate with secondary care to develop, review, and implement policies, integrated care pathways, guidelines, and patient information materials that reflect current trends in nursing and the NHS.

Work across primary and secondary care settings to facilitate seamless patient care transitions.

Execute a high level of clinical skills in assessment, diagnosis, or treatment.

Educate and support community staff regarding all aspects of Lymphoedema and chronic edema management.

Provide specialized knowledge and guidance to colleagues and patients within a multidisciplinary environment.

Act as a deputy for the line manager as necessary.

Assist the line manager in managing and developing the Lymphoedema Service.

Engage in the Trust's organizational development initiatives to transform care delivery methods, ensuring alignment with the Trust's values and vision.

Participate in clinical supervision in accordance with Trust policy.

Receive and triage direct referrals to assess patient needs with support from other clinicians in the team.

Enhance personal knowledge and expertise in the specialty related to patient treatment, developing individualized care plans and health promotion strategies.

Collaborate with safeguarding teams and community health professionals to support vulnerable patients.

Ensure that therapeutic interventions are clinically effective and evidence-based.

Respond to patient and service needs, organizing personal and team workloads as required.

Actively engage in health promotion and the prevention of illness among patients, caregivers, and colleagues.

Seek opportunities to enhance patient care based on identified needs.

Facilitate meetings and working groups as needed.

Provide information to the line manager to assist in service development.

Manage personal workload and, when necessary, oversee the workload of junior staff.

Initiate and participate in audits related to all aspects of Lymphoedema management to ensure high standards of clinical practice across the Trust.

Maintain a database of clinical incidents related to the service, producing quarterly reports in collaboration with the line manager.

Participate in relevant committees and groups as required by the line manager.

Document activities to generate reports demonstrating the outcomes of the Lymphoedema service.

Under the guidance of the line manager and in collaboration with multidisciplinary teams, provide high-quality skilled care in Lymphoedema management across various settings, utilizing evidence-based practices and adhering to ELHT national policies and standards.

Engage in audit and monitoring mechanisms and product evaluations within the service.

Maintain clear and concise records of individual patient assessments, advice, and anticipated outcomes in electronic notes.

Ensure proper disposal of clinical waste in accordance with infection control policies.

Remain attentive to patients' needs and organize personal workload accordingly.

Monitor patients' conditions and report significant changes and concerns to the line manager and other professionals as appropriate.

Adopt a holistic approach to patient care, treating individuals with respect and dignity in their homes or community settings.

Collaborate with nursing and non-nursing colleagues to foster positive working relationships for the benefit of patients.

Attend and contribute to staff meetings and working groups to identify areas for improvement and ensure objectives are met.

Partner with the line manager to support the development of and participate in patient support groups.

Maintain a flexible approach to the working day and week to meet patient and service needs, including potential cross-site working as required.
