NELFT NHS Foundation Trust is seeking a highly skilled and dedicated NHS Specialist Nurse to join our Community Neurological Nursing Team.
About the Role:The successful candidate will be responsible for providing high-quality care and support to individuals with Multiple Sclerosis, Motor Neurone Disease, and Parkinson's disease in the Havering and Barking & Dagenham areas.
Key Responsibilities:- Develop and implement individualized care plans for patients with neurological conditions, promoting seamless care and a patient-centered approach.
- Work collaboratively with the Community Rehabilitation Service and local care teams to ensure comprehensive support for patients.
- Provide expert advice and information to carers and healthcare professionals on aspects of care and support.
- Ensure effective risk management through accurate reporting, assessment, and control of risks, and maintaining accurate records.
- Current Registered Nurse qualification (Part 1 of NMC register).
- Cert, Diploma, or First-level Degree in Nursing.
- Working towards or be a Non-Medical Prescriber.
