We are looking for a Senior Clinical Practitioner to enhance our Urgent Community Response (UCR) initiative in South Bristol.

The ideal candidate will serve as a Senior Clinical Practitioner, playing a crucial role in the Urgent Community Response pathway aimed at minimizing unnecessary hospital admissions and promoting effective admission avoidance strategies.

This position demands advanced clinical assessment capabilities and the application of non-medical prescribing qualifications to provide medical support for complex patients in the community, who may present with a variety of acute medical issues.

Given the qualifications required for this role, specifically the Non-Medical Independent & Supplementary Prescriber status (or the willingness to pursue this qualification), we are currently unable to consider applications from Occupational Therapists.

Key Responsibilities

As a Senior Clinical Practitioner, you will operate independently within the Community Urgent Response pathway, which is part of our Integrated Neighbourhood Teams.

Your responsibilities will include comprehensive history taking, clinical evaluations, diagnoses, and implementing a variety of interventions for patients with acute undiagnosed conditions and managing chronic long-term illnesses.

The successful candidate will exhibit critical thinking in clinical decision-making, identify service gaps, and contribute to service enhancement and development in alignment with national and local strategies.

Collaboration with multidisciplinary team members will be essential to establish a holistic approach to healthcare management for individuals with complex care requirements.

The post holder will offer clinical leadership and ensure the delivery of effective and safe clinical practice standards across various community services.

This position is not classified as an Advanced Practice role aligned with the HEE Multi-professional Framework for Advancing Practice. These roles are currently under evaluation and may undergo a name change in the near future. This is due to the fact that these positions do not necessitate candidates to operate across all four pillars of Advanced Practice to MSc level and are therefore categorized at AFC Band 7. However, they do require a skill set operating at or towards MSc level in the Advanced Practice Clinical Pillar. This role will not qualify for training funding for an MSc in Advanced Practice, although training may be supported based on individual learning needs identified through Sirona CPD funding application processes.

Qualifications

Essential

  • Non-Medical Independent & Supplementary Prescriber or willingness to undertake the Pharmacology and Prescribing Module at Masters Level.
  • Completion of the Physical Examination and Diagnostic Reasoning Module (or equivalent) at Masters Level or willingness to undertake.

Desirable

  • Possession of or progress towards a practice educator qualification or equivalent.
  • Teaching and assessing qualifications/experience.
  • Evidence of a Master's level clinical qualification.
