Shift Schedule: 8 p.m. to 8 a.m. & 8 a.m. to 8 p.m.

Live Connected is an innovative segment of the Connected Health group, dedicated to delivering exceptional care to adults facing mental health challenges, learning disabilities, dementia, and complex needs through supported living.

KEY RESPONSIBILITIES:

  • Assist with daily living activities as agreed upon with the service user, their family, and involved professionals.
  • Adhere to the policies and procedures of Connected Living at all times.
  • Collaborate with care and risk assessments tailored to each service user's requirements.
  • Conduct Health and Safety checks in accordance with established policies and procedures.
  • Be prepared to work independently in alignment with lone worker risk assessments.
  • Ensure safe medication delivery and ordering to service users' residences.
  • Empower service users to express their preferences regarding task execution.
  • Promote choice and independence for the service users we support.
  • Manage service users' behaviors in the least restrictive manner possible.
  • Complete daily notes, handovers, and any relevant documentation by the end of your shift, ensuring high-quality handovers for the incoming shift.
  • Collaborate as part of a team and the wider multidisciplinary team to support clients' emotional and physical wellbeing.
  • Foster and maintain professional relationships with service users and the relevant multidisciplinary team to ensure positive outcomes.
