About the RoleWe are seeking a highly motivated and compassionate Support Practitioner to join our team at Strathbungo Housing Support Service.
As a Support Practitioner, you will work on a 1:1 basis with individuals who are deafblind and have diverse, complex support needs, providing tailored support and engaging activities to promote their independence and well-being.
Key Responsibilities:
- Provide person-centred care and support to individuals with complex needs, including personal care, housework, and medication administration.
- Liaise with families and other professionals to ensure seamless care and support.
- Promote independence and individuality, working with individuals to achieve their personal goals and ambitions.
- Record daily tasks and maintain accurate records, ensuring effective communication with the team and stakeholders.
- Work collaboratively with the team to provide 24/7 support, including sleepovers and weekend shifts.
