As a Crisis Practitioner, you will play a key role in delivering our service, working collaboratively with a multidisciplinary team to provide crisis mental health assessment and interventions for Children and Young People.
Key Responsibilities- Conduct comprehensive mental health risk assessments and develop crisis intervention plans
- Provide 7-day follow-ups and safeguard young people as needed
- Liaise with other services and external agencies to ensure effective pathways of care
- Work collaboratively with the team to promote safety and support young people and their families
- Professional registration with HCPC, Social Work England, or the NMC
- Relevant experience of assessing and managing risk within a mental health or social care setting
- Ability to work collaboratively as part of a team and prioritize work to meet the needs of the service
