About the Role
We are seeking a highly skilled and experienced Clinical Lead to join our team at Cheshire and Wirral Partnership NHS Foundation Trust. As a Clinical Lead, you will be responsible for providing clinical leadership and expertise in dementia care, working closely with our multidisciplinary team to deliver high-quality patient care.
Key Responsibilities
- Provide clinical leadership and expertise in dementia care, working closely with the ward team to promote a high standard of patient care.
- Develop and implement evidence-based interventions to improve patient outcomes and care pathways.
- Collaborate with the multidisciplinary team to ensure holistic patient-centered care planning.
- Assess, evaluate, and advise staff on care planning and discharge planning.
- Develop and implement strategies to improve efficiency and patient care.

Person Specification
- NMC Registered Nurse RMN or RNLD.
- Evidence of Continued Professional Development.
- High level of clinical expertise.
- Understanding and appreciation of national policy and health/social care issues.
- Clear understanding of social inclusion/equality/diversity.
- Ability to work collaboratively across organisational boundaries.
- Knowledge of cultural diversity within the healthcare setting.
- Awareness of current mental health issues, debates, and practice.
- Knowledge and understanding of physical health needs.
- A willingness to undertake further training.
